Unified Sports

Unified Sports aims to foster equality and social inclusion by joining students with and without disability in athletic competition. By doing so, the program generates both friendship and understanding between students at every level. Unified Sports is open to students of all academic and athletic ability, and seeks to provide an opportunity to educate our students about social inclusion. 

Unified Sports teams consist of an even mixture of students with and without disabilities. Teams are designed to create a level playing field of ability, thereby keeping games challenging and exciting. By practicing and playing together, it is the mission of Unified Sports to grow a culture of inclusiveness within our schools through athletics. By forming meaningful friendships and creating opportunities for genuine athletic experiences, Unified Sports aims to help bring the world into the future of inclusion. 

High school Unified Partners who would like to participate in an NJSIAA interscholastic Unified Sports competition must meet all eligibility requirements outlined by the NJSIAA and his or her school district.  Please see the information below to get your student registered for the Unified Sports season.

INFORMATION AND REQUIRED FORMS:

The following forms must be turned in as a hard copy to the school health office: 

First Time Athlete and/or First Sport Season of the School Year (non-current physical)

Current Physical - First Sport of the Year


Health History Update
(required for valid school year physicals older than 90 days prior to start of sport.)

NJSIAA Steroids Consent to Test Form

Concussion baseline testing:  must be taken before the sports season starts:
9th, 11th and New Athletes  


Please remind your student athlete to check in with the nurses prior to the start of a sport season to ensure all paperwork is in order. All students should check in with the nurses to determine what paperwork is needed and to obtain sports forms for physicals.  Appointments will not be made over the phone as pre-physicals with the nurses must be completed before a student is scheduled to see the school doctor.
